Initial Psychiatric Evaluation (ages 18 and above)

A 60-minute comprehensive initial psychiatric assessment to go over your mental health needs and history while addressing any concerns/questions you might have to determine if I can provide the appropriate ongoing psychiatric services for your mental health needs.

The initial evaluation does not initiate a professional relationship, both parties must agree to work together after the initial evaluation for a professional relationship to be initiated.

Follow up Visit (ages 18 and above)

$300

A 30 minute follow up visit to review ongoing progress and discuss medication management.

Frequently Asked Questions

Do you accept insurance?

Currently, I do not accept insurance. However, some insurance companies provide out of network benefits. You can find out more by contacting your insurance company and asking about your "Out of network mental health benefits".

While we cannot make any guarantees that your insurance company will reimburse you, if you do wish to submit a claim, I'd be happy to provide a detailed receipt or a super bill upon request.

What form of payment to you accept?

All fees are due at the time services are rendered. I accept debit and credit card payments through the patient portal which utilizes a PCI protected and HIPAA secure card-on-file payment service. Select FSA/HSA cards (Visa, MasterCard, Discover and American Express) are also accepted.

How often will I be seen for an appointment?

This would depend on your particular case and individual needs; however, most patients are seen every 1-3 months.

What is your cancellation policy?

I consider time to be very valuable and therefore I aim to respect and value your time, I only ask that you do the same in return.

I ask that you cancel or reschedule an appointment at least 1 business day (24 hours) prior to your appointment time. Full fees are charged for missed appointments or ones cancelled within 24 hours of appointment time.

Do you provide disability assessments?

I do not provide long-term disability assessments or evaluations for legal purposes. Short-term leave requests may be considered on a case-by-case basis for long-term established patients (patients seen regularly for at least 6 months). I do not provide paperwork or evaluations for emotional support or service animals.

Do you provide emergency services?

My solo private practice is meant to provide non-urgent, outpatient psychiatric services. Due to the nature of my single physician practice, I am unable to respond to emergent requests. Furthermore, my practice is not equipped to manage psychiatric conditions that predispose patients to recurrent mental health crises requiring frequent urgent psychiatric care or intensive services.

For medical and/or psychiatric emergencies, please call “911” or go to the nearest emergency room.

How do I schedule an appointment?

You can request a consultation online and fill out a pre-screening form here. We will then review the information and get back to you. Once the request has been accepted, we will send you an invitation to schedule an appointment through the patient portal where you can access our calendar. We will also send you the required forms that need to be filled and signed before our appointment.
